Important Note: Fill out documents carefully and sign each area that you have completed. You must submit the forms to GRACE Staffing by e-mail. In order to send them by email you must fill out the form and be sure to SAVE AS. If forms are being filled out within a browser, click the print icon, then click the “Save as PDF” option within the Print Preview Window. Then you attach the file to your email. If you want to send them by mail, simply print them out after you have completed them.
